Ken,

I think there's some confusion here, maybe in the way your previous question
was asked and answered.

Your fighter can take two attacks against someone on their turn, provided
that he's got them marked, and they're making a type of attack that provokes
an Opportunity Attack.

Otherwise, he can only make one attack because he only gets one Immediate
action per round.

To explain, think of it like this. Your fighter has attacked an orc, and has
him marked. The orc, on its turn, then makes a ranged attack against someone
other than the fighter. The fighter would get one Immediate interrupt basic
melee attack from Combat Challenge (because the marked orc attacked someone
other than him) and one Opportunity basic melee attack (because the orc used
a ranged attack, provoking an opportunity attack from him).

Does that make a bit more sense? If the orc shifts, the fighter can get his
Combat Challenge immediate interrupt attack, but then if the orc uses a melee
attack, the fighter won't get a second attack because he's already used his
immediate action.
